JFS At Home Strengthens Partnership with Baptist Health Through “Hospital-To-Home” Initiative

On Thursday, May 14, representatives from JFS and JFS at Home met with a group of registered nurses, social workers, and discharge planners at Baptist Health to strengthen collaboration around patient care and successful hospital discharges.

The presentation team included Alice Tennis, Senior Vice President of Senior Services at JFS; Rafie Merzel, RN, of JFS at Home; Ed Levine, Chair of the JFS at Home Board; and fellow leadership representatives. Together, they shared information about JFS Senior Services and JFS at Home’s private duty home care offerings, highlighting how the organizations can serve as trusted partners in ensuring safe and supportive transitions from hospital to home.

A key focus of the discussion was the promotion of the JFS at Home “Hospital-to-Home” program, designed to provide patients with immediate post-discharge support and connect them to additional services as needed. The program offers families peace of mind during what can often be a stressful transition, while giving them valuable time to plan for long-term care and support.

The presentation was very well-received by the Baptist Health team, and the meeting marked an important step toward building a stronger collaborative relationship. Following the session, the JFS team was invited to provide periodic updates and educational information through Carin to continue supporting the hospital staff and their patients.
