
Hospital-to-home transition care is rapidly emerging as one of the most important areas within senior services, and JFS at Home is positioning itself at the forefront of this growing need with a compassionate, highly personalized approach designed to support both older adults and their families during vulnerable recovery periods.

“Having a loved one discharged from the hospital or rehabilitation center is one of the most vulnerable and stressful situations a family can experience,” said Gordon. “Families are suddenly faced with making significant decisions while trying to navigate unfamiliar medical, logistical and emotional challenges. They shouldn't have to do that alone."
“Our role is to develop a customized transition plan that removes as much of the stress as possible, ensuring a safe, smooth recovery at home while giving families the time and peace of mind to come together, evaluate long-term options and make thoughtful decisions without feeling pressured during a crisis,” she added.
The program focuses on creating what leadership describes as “safe and successful discharges,” ensuring that clients do not return home alone or unsupported during a critical transition period. Caregivers can assist with everything from picking up medications and groceries to helping clients settle comfortably at home, monitoring recovery needs, coordinating follow-up therapy appointments and providing hands-on care tailored to the individual’s condition.
Importantly, JFS at Home recognizes that no two discharges look the same. Some individuals may require around-the-clock support immediately after hospitalization, while others need only overnight supervision or a few hours of assistance each day.
“To simplify decision-making during stressful moments, JFS at Home has developed easy-to-understand transitional care packages that combine nursing assessments, care coordination and short-term caregiving support into flat-rate recovery plans for the first several days after discharge,” said Rafie Merzel, a registered nurse who has been with JFS at Home since its inception.
“The goal is not only to support the client medically and physically, but also to give families time to breathe, regroup and determine longer-term next steps without feeling pressured to make immediate decisions in the middle of a crisis,” Merzel added.
What distinguishes the JFS at Home approach is its emphasis on reassurance, responsiveness and personal connection. Leadership emphasized that families are often emotionally overwhelmed during these moments and are searching for more than practical caregiving help. They want to know someone trustworthy is available, accessible and capable of managing the details that suddenly feel unmanageable.
“JFS at Home believes its small, highly connected care team offers a meaningful advantage in that regard. Families know who they are working with, can easily reach someone when concerns arise, and have access to nursing guidance when needed. This relationship-centered model helps build confidence at a time when many families feel exhausted, uncertain and emotionally stretched thin,” said Gordon.
